1. Who we are and what this policy covers
Her Wellth is operated by Alena Dushenko as an individual using the trade name Her Wellth. This policy applies to her-wellth.com, free assessments, contact forms, email subscriptions, purchases, account activity, intake forms, submitted laboratory and wearable information, support messages and client deliverables. 2. Categories of personal information
Depending on how you interact with the service, we may receive your name, email, account credentials or authentication identifiers, approximate location or service state, age or birth-year information, health and wellness goals, reported symptoms, nutrition and lifestyle history, medications and supplements you disclose, reproductive or postpartum information, uploaded laboratory records, wearable-device exports or summaries, purchases, messages, and records of the consent selections you make. Our website and service providers may collect ordinary device and site-usage information such as browser details, IP address, cookies, referral pages, security events and timestamps. Our payment processor ordinarily handles full payment-card details; we may receive payment status, billing information and transaction identifiers.
3. When we collect information
Information may be submitted through the free assessment, which requires an email address; voluntary marketing opt-in is a separate choice. Paid clients may submit expanded sensitive information through the Intake Form, uploaded laboratory results and optional wearable files. The Contact Us page collects the details entered in its form for reply and administration. We collect only information needed for the purpose disclosed at the relevant point and will request express consent for processing sensitive health information where required.
4. Purposes of use
We use the information to deliver free assessment summaries; process purchases; check service eligibility; administer accounts, optional laboratory workflows and client support; review materials and prepare personalized educational plans; respond to inquiries; maintain transaction and consent records; detect misuse; improve reliability; comply with legal obligations; and send marketing only where permitted and separately requested. Health information obtained for a program is not used to create targeted advertising audiences.
5. Human review and software assistance
Her Wellth may use approved software to organize submitted information, calculate nonclinical estimates and draft educational material. Final client-specific analyses and recommendations are reviewed and approved personally by Alena Dushenko before release. No website-generated material is intended as an automated diagnosis or emergency screening system. We do not disclose identifiable sensitive health information to a generative AI provider unless that use is explained to the individual, appropriate safeguards exist, and required permissions are obtained.
6. Recipients and independent third parties
Information may be shared when reasonably necessary with website hosts and forms, protected storage, email delivery, account authentication, payment processors, customer-support tools, and laboratory-ordering or clinical providers when you choose testing and authorize a permitted exchange. These parties perform distinct functions and may apply separate terms and privacy notices. We use Goodlabs as an available laboratory-ordering platform through a registered practitioner account; this statement does not imply a formal partnership, employment or agency arrangement. Specimen collection may be scheduled at available Quest Diagnostics locations. We may disclose information where legally required, in connection with professional advice or a lawful business transfer, or at your direction. We do not disclose identified health records to advertisers for targeted ads.
7. Marketing choices and cookies
Service or order-related communications are distinct from optional promotional emails. Marketing consent is not a condition of assessment or purchase, and you can unsubscribe at any time using a message link or by emailing us. Cookies essential for site operation may be used; any nonessential analytics or advertising technology must be configured in accordance with applicable consent requirements and must not receive sensitive intake answers, lab records or wearable exports.
8. Security and retention
We seek to maintain administrative, technical and organizational safeguards appropriate to the sensitivity of information and to limit internal access to those who need it. No transmission or storage system is risk-free. Information is kept only for service delivery, reasonable recordkeeping, legal compliance, dispute handling, and account security, then deleted or de-identified where practicable subject to legitimate retention obligations. Exact storage services, access controls, retention schedules and backups depend on the live production configuration and must be implemented consistently with this notice; this statement is not a representation that an unverified security feature is already deployed.
9. Privacy requests
To request access, correction, deletion, portability where required, withdrawal of a consent, or information about our practices, email hello.herwellth@gmail.com with the subject "Privacy Request." We may verify your identity before acting and may retain information that we are lawfully required or permitted to keep. Any rights and response deadlines under applicable state or federal laws will be honored when applicable.
10. Minors, external links and changes
Paid programs are for adults age 18 or older, and we do not knowingly invite children to submit health information through the client intake. External laboratories and payment sites apply their own notices. We may update this policy for future processing; material changes will be communicated as required before new use of affected information. This policy is not a representation that Her Wellth is a HIPAA-covered entity; any applicable HIPAA or federal or state breach-notification duties will be assessed and met as required by law.
